CHW Surveyor - Part-time/Temporary (link here for PDF)
The Chicago Community Health Worker/Promotor Survey and Mapping Project is preparing to launch a pilot in Rogers Park, and is hiring a surveyor for the pilot. The surveyor will contact and interview community health workers, promotores de salud, and CHW/promotor program supervisors, in order to collect information about the work of Chicago community health workers; find out what Chicago CHWs feel are the most important health needs in the communities where they work; collect information about the service areas of Chicago’s community health workers; and track the specialties, demographics, and training needs of Chicago CHWs in order to guide the Chicago CHW Local Network and improve the service of City and community agencies to CHWs and Promotores. The project collaborators hope to use the results of the Rogers Park pilot study to develop a Chicago-wide inventory project.
